The Shea Ear Clinic regularly accepts referrals from referring physicians, including primary care providers. Physicians interested in referring patients may do so via phone, fax, mail or email.
Please print and use the Shea Ear Clinic’s Physician Referral Form.
If an insurance provider requires a referral number, the referring physician should obtain this number as well and submit it with the referral.
Please fax the completed physician referral form to 901-683-8440 to the attention of “Appointments” or you may scan and email the form to firstname.lastname@example.org.
Referrals should include all pertinent patient information including but not limited to office notes, operative reports, test results, diagnostic results like CT scans or MRI’s, and reason for referral. We respectfully request you forward your patient’s medical records as soon as possible so that we may route them appropriately.
